Our Advisory Board Members facilitate the organization and execution of quarterly round table sessions for members within the sector they represent. The purpose is to connect with your peers- reviewing barriers, challenges or opportunities for key topics such as care navigation, coordination & transitions. Members are also encouraged to discuss other relevant topics identified by those in attendance, our council or other trusted sources.
Topics of discussion then cascade to our council for review which can be addressed broadly for all members at our General Assemblies.
Round table sessions are the newest endeavor of ours in our efforts to collaborate for care continuity.

We invite current & prospective members to assemble quarterly at one of our Partner Hospitals. This meeting is a convening of professionals dedicated to efficient, effective care coordination across various healthcare settings. This gathering unites the spectrum of care levels and professional roles including liaisons, hospital case management + social work, alongside those managing care teams and facilities in post-discharge settings.
This meeting is tailor-made for professionals involved in care coordination, especially those navigating the complexities of patient transitions from hospital to post-acute care environments.

Case Management Overview: An overview of the health systems case management model, including any innovative or effective approaches. Case Management leadership &/or management contacts and primary role or responsibilities.
Post-Acute Care Needs: A assessment of the current landscape and challenges in post-acute care as identified by the presenting hospitals case management & social work dept. What the underserved needs, where are services needed?
Post-Acute Discharge Analytics: A comprehensive review of discharge trends and data broken down by hospital sites, ideally from the last four full quarters where data is available. e.g. % of patients discharged with home care vs SNF vs hospice by major payor category (Medicare A, Medicare C, Medicaid, Commercial)
High-Performance Post-Acute Network: Insight into the high-performance networks in post-acute care. Goals, outcomes, eligibility.
Additional Highlights: Presentation of other significant achievements or information from the host hospital system as desired.
